As the first private step in tackling an issue he has been silently living with for months or years, a man often looks for an online course on erectile dysfunction. I understand the attraction. It’s much less exposing to present structured information on your own schedule than it is to sit across from a stranger and describe what happens, or doesn’t happen, in the bedroom. Such a desire for privacy and control is reasonable and not a way of avoiding treatment. It can be a valid gateway into treatment, provided that the course is based on sound clinical principles and that the reader is aware of its capabilities and limitations.

In this article I look at what online courses for erectile dysfunction really offer, what the research says about psychological ED treatment, what makes a well-designed course different from a superficial one and when a course alone is not enough.

An erectile dysfunction online course is a structured, self-guided educational program that teaches the psychological and behavioral skills used in sex therapy to address erectile difficulty. The course is generally delivered through video modules, written material and guided exercises, rather than live sessions with a clinician. The most successful programs use the same frameworks as in-person sex therapy: cognitive behavioral therapy (CBT), psychoeducation about the erection process, and structured behavioral exercises.

The main difference between a course and therapy is not the content, but the manner in which it is delivered. Therapy is interactive: a therapist reacts to a client’s statements in the moment, asks probing questions, and identifies issues that a client may not think to mention. The course provides the same underlying principles in a fixed sequence without that individualized feedback loop.

It influences the way a man communicates with a partner, the extent to which he avoids intimacy altogether, and how a partner perceives what is going on. A good course will address this head on, providing language and structure for talking about ED with a partner rather than leaving that conversation to chance.

What Online Courses Can’t Teach

When there is a physical component to erectile dysfunction, a course is not a substitute for medical evaluation. In such cases, couples therapy is often necessary to address significant relationship conflict, unresolved resentment, or a breakdown in emotional intimacy that requires the responsiveness of a clinician working directly with both partners, rather than having one partner work through the material alone.

Online Course or One-on-One Therapy: Which is Better?

If the erectile difficulty is mostly situational, related to anxiety or particular situations, and there is no major physical health concern or relationship crisis driving it, a course is a reasonable place to begin. When a man has tried self-directed strategies and made no progress; when a physical cause has not been ruled out; or when the relationship itself requires direct clinical attention, one-on-one therapy, or a combination of the two, is the better path.

Many men do both. They take a course to establish a basis of understanding and basic technique, and then take specific, harder to shift patterns to a therapist, rather than starting therapy from a blank page.

Such behavioral change tends to build up gradually rather than to be resolved overnight. Skipping steps in the sequence usually tends to slow, rather than speed up, progress.

If erectile dysfunction is caused by a physical cause is an online course sufficient?

No.  If there is a physical or medical reason for erectile dysfunction, such as a vascular, hormonal or cardiovascular problem, then a course that works with psychological and behavioural patterns will not solve it on its own.
